Mandated Health Screenings
Students are required to have a physical completed in Kindergarten or 1st grade, 6th grade, and 11th grade.Hearing Screenings
Hearing screenings are required for grades K, 1, 2, 3, 7, and 11.Dental Screenings
Dental screenings are required in the following grades: Kindergarten or 1st grade and 7th grade. The mobile dentist comes two times a year to school for routine care. Students may be seen as long as the form is completed.
For the 2024-2025 school year, dental screenings will be provided through the Smile Pennsylvania Program Mobile Dentists on October 23-24, 2024, and April 24-25, 2025. Dental forms will be sent home with all students and MUST be completed and returned to receive the FREE dental screening. Parents can also complete the form online by going to https://mobiledentists.com/for-parents/.
Vision Screenings
Vision screenings are required each school year and are completed by the school nurse.
Vaccinations:
Students are required to have the following vaccinations unless they have an exemption.
Entering Kindergarten:
4 DTAP (1 dose on or after the student's 4th birthday)
4 POLIO (4th dose on or after the student's 4th birthday and at least 6 months after the last dose)
2 MMR
2 Varicella
3 Hepatitis B
Entering 7th Grade: (By the first day of school including required Kindergarten vaccines)
TDAP
MCV
Entering 12th Grade: (By the first day of school unless the student received a previous dose at age 16 or older)
MCV (2nd dose)
