Returning Substitutes

All returning substitute counselors, therapists, and Behavior Interventionists please fill out the Returning Substitute Form. This form is required annually in order to have updated information to continue subbing.

If you are interested in substituting as a aide, counselor, therapist, or Behavior Interventionist for the next school year and haven't worked at least one day in the current school year, please submit the above survey online and submit new clearances. Clearances are listed below:

• On-line PA Child Abuse History Clearance Application
• On-line Pennsylvania Criminal Record Check Form
• Arrest/Conviction Report – Act 24
• FBI Fingerprinting Background Check

  • IU Service code - 1KG6S7
